Medical ID Jewelry Information
Patients with chronic kidney disease, those on dialysis, transplant recipients and living donors are all eligible to receive free medical identification jewelry. Download the Medical ID Jewelry Order Form for Chronic Kidney Disease patients, those on dialysis, transplant recipients and living donors. Allow 2-4 weeks for processing.
Do Not Resuscitate (DNR) Medical ID Jewelry
Dialysis social workers can order DNR bracelets for Wisconsin dialysis patients through the National Kidney Foundation of Wisconsin.
Orders must be made on the DNR Medical Identification Jewelry Request Form provided by the National Kidney Foundation of Wisconsin.
Orders must include a copy of the doctor’s order (Emergency Care Do Not Resuscitate Order Form F-44763).

Please contact the National Kidney Foundation of Wisconsin office so that we can customize an order form that you or your patients can use. All materials are provided free of charge to individual patients.
